Office Policy

Cancellation Policy:

Your treatment at Revive Med Spa are reserved exclusively for you. Please kindly allow 24 hours (1 business day) to notify us of the cancellation. All cancellations with less than 24 hours notice are subject to a $35 cancellation fee. Revive Med Spa requires a credit card to reserve your appointment ; This courtesy enables us to compensate our employees for their time and maintains a higher availability of our time for you as well as others.  

No Show / Late Arrival Policy:  

A "No Show" is someone who misses an appointment without canceling it at least 24 hours in advance or who fails to keep a scheduled appointment. In the event a 24 hour notice is not given, a fee of $35 will be charged to your credit card on file. Treatment can-not be guaranteed if you arrive more than 15 minutes late. Clients who cancel, reschedule or arrive late frequently will be requried to schedule their appointments on the day they want services, rather than being offered the ability to schedule in advance. 

Refund Policy:

We offer goods and services that are irrevocable. Therefore, we do not issue refunds for any product or service that has been injected or used in your treatment.
